HealthHIV’s Pozitively Aging program will drive care optimization for people aging with HIV. Our emphasis is communities of color, addressing care optimization through relevant and timely provider and consumer education, patient empowerment on access to care, and impactful advocacy on numerous policies to improve access to and utilization of services. The program includes an environmental assessment, dissemination, engagement/collaboration and evaluation, moving from coordination to optimization.
